Guidance for application
Every organisation applying to become an SQA Centre must read the following guidance before applying.

Forms for application
Every organisation applying to become an SQA Centre must complete the following forms. Select the relevant qualification form(s) to you, depending on the qualification you would like to deliver.
- Systems approval form (1.49 MB)
- Qualifications approval form - National Qualifications (427 KB)
- Qualifications approval form - All qualifications (except National Qualifications) (317 KB)
- Staff supporting document (710 KB) - please complete one copy for each staff member who will assess or internally verify the qualifications you wish to offer

- Apply to offer an additional qualification in the same subject area (541 KB)
